Please arrive 15 minutes early for your appointment. You may want to arrange to have your previous records sent to us prior to your appointment by contacting the facility you visited.
To speed up the check-in process we use an electronic check-in kiosk. The kiosk is user friendly and convenient. You may check out this video for information on how these electronic check-in stations work.
When you come for your appointment, it would be helpful to bring the following information
- Photo ID
- Health insurance card
- If applicable, copies of surgical records, medical records, X-rays, MRI’s, CT scans, bone scans or ultrasounds
- A list of all medications that you are currently taking
- Your claim number if it is a workers’ compensation injury
